Referral

To make an appointment you will need a referral from your GP

Patient registration form

You will be asked to complete a patient registration form. This can be done prior and emailed to us or filled in on the day.

Imaging

Please bring any X-rays, ultrasounds, CT or MRI scans with you to your appointment

​

Insurance details

Please have your insurance details or workcover/TAC claim number with you on the day
